Why the RN license matters in California
California does not license the job title "care manager." Anyone can use it. What the state regulates is the underlying activity, assessing a patient's condition, deciding whether what you are seeing is abnormal, and administering or overseeing medical treatment. That activity is defined as the practice of nursing under California Business and Professions Code section 2725, and only a licensed Registered Nurse, or a Licensed Vocational Nurse acting under an RN's or physician's direction, may legally perform it.
Section 2725 gives an RN four specific authorities:
- Direct and indirect patient care, including safety, comfort and disease prevention
- Administering medications and treatments a physician has ordered
- Performing skin tests, immunization techniques, and drawing blood
- Observing signs and symptoms, determining whether they are abnormal, and acting on that determination through reporting, referral, standardized procedures or emergency measures

Do this before you hire anyone, including us. Every California RN and LVN license is searchable, free, through the state's own systems. Search by license number where you can, because common names return more than one result.

A clean license lookup tells you the person holds the credential they claim. It does not tell you they are a fit for your family. Ask to see a sample care plan, ask who covers when they are unavailable, and ask whether they take referral fees from the facilities they recommend.

The first week is the assessment and the written care plan. The second is usually triage: the two or three things that are actively unsafe get fixed first, a medication list nobody has reconciled, a fall risk in the bathroom, a specialist appointment that should have happened months ago. RN care management is private pay. Medicare does not cover private care management, RN-led or otherwise. Its home health benefit covers part-time skilled care under a physician's order, and it does not include care coordination as a standalone service. Is an RN care manager the same as a geriatric care manager?
The work is the same: assessing the situation, building a written care plan, coordinating providers, and adjusting the plan over time. The credential is what differs. Geriatric care managers come from several backgrounds, nursing, social work, and gerontology among them. A nurse care manager is a licensed Registered Nurse doing that same work. Ask what license the person actually holds, because the job title alone does not tell you.
Does Medicare pay for an RN care manager in Orange County?
No. Medicare does not cover private care management, RN-led or otherwise. Medicare's home health benefit covers part-time or intermittent skilled visits, generally up to 8 hours a day and 28 hours a week, for homebound patients under a physician's order, and it does not include care coordination as a standalone service. What can a registered nurse do that a non-licensed care coordinator cannot?
California Business and Professions Code section 2725 gives an RN the authority to observe signs and symptoms, decide whether they are abnormal, and act on that decision through reporting, referral, standardized procedures, or emergency measures, without waiting for someone else's order in the moment. A non-clinical care manager who notices a concerning change still has to refer it to a nurse or physician before anyone can act on it. That referral step costs time in situations where time matters.
