Where good hospital coverage still leaves a gap at home
Ontario has hospital care close by. Kaiser Permanente Ontario Medical Center is on South Vineyard Avenue, and San Antonio Regional Hospital, based in neighboring Upland, runs an outpatient location in the city. Strong hospital coverage does not translate into hours of skilled nursing at home, and that is where a family can discover the gap.
Kaiser Permanente Ontario Medical Center
Kaiser's Ontario campus is a medical center at 2295 South Vineyard Avenue, with emergency, cardiology, surgery and obstetrics. It handles the acute episode. The hospital does not send a nurse home with you for the twelve hours a day afterwards when a recovery is genuinely fragile.

Check what your health plan covers at home before discharge, because continuous private duty nursing is a different service from the admission. Medicare does not pay for 24-hour-a-day care at home (Medicare.gov). Learning that in week one produces a very different plan than learning it in week three.
San Antonio Regional's Ontario site is urgent care, not the hospital
San Antonio Regional Hospital is a nonprofit hospital with surgical, cardiac and emergency care at its 999 San Bernardino Road campus in Upland. Its Ontario Urgent Care Plus location on North Mountain Avenue handles urgent visits and outpatient behavioral health, not surgery or inpatient stays, so a family discharged from the Upland campus is coming home from outside Ontario.

Ontario families come home from inpatient procedures at Kaiser Permanente Ontario Medical Center, outpatient elective cases at Ontario Advanced Surgery Center on West 6th Street, or San Antonio Regional Hospital's campus in Upland. What a patient needs at home on day one depends on which of those it was, and on how physically demanding daily life is otherwise. We plan coverage with you and your surgeon around what the procedure actually requires, so you are not guessing. All care is private-pay, coordinated with your surgeon's discharge plan rather than replacing it.
A daytime visit
Best for lower-acuity outpatient procedures, the visit type suited to same-day elective cases done at Ontario Advanced Surgery Center, an ambulatory surgical center on West 6th Street. A licensed nurse (RN or LVN), under RN oversight, checks vital signs, reviews the discharge instructions with the family, sets up medications, and confirms wound care is being followed correctly. Also used as a standalone wellness check visit outside of a surgical context.
The first full day home
Built for the day a patient comes home after a standard procedure, when the hospital's own care team is no longer watching. Covers vital sign monitoring, pain management coordination with the surgeon's office, mobility assistance, and medication administration per physician orders through a full first day home.
A longer day for higher-acuity cases
For higher-acuity cases, such as spine surgery, joint revision, or shoulder, back, and knee procedures, where a shorter hospital stay does not mean a lighter clinical need. Covers more intensive monitoring, drain management, and closer pain titration coordination with the surgical team across a longer day.
Overnight
For patients who want a private nurse at home overnight rather than an unattended first night. A licensed nurse (RN or LVN), under RN oversight, stays through the night for symptom monitoring, medication timing, and mobility assistance if the patient needs to get up.
Around the clock
Matched to hip fracture recovery and to older adults recovering alone in Ontario's established neighborhoods near downtown, when working-age family members cannot be home during the day to provide it themselves. Continuous coverage by a licensed nurse (RN or LVN), under RN oversight, so care never steps down to unattended hours.
Several days or longer
For complex spine, joint revision, or longer downtime recoveries, and for out-of-town family members flying into Ontario International Airport who need coverage bridging the days before they arrive or after they leave. Scheduling and staffing are built around the individual recovery plan rather than a fixed block, and we confirm them with you after intake.

The hospital with the emergency department, operating rooms, and inpatient surgical units is San Antonio Regional Hospital's main campus in neighboring Upland.
How many hours can we get, and is 24-hour care possible?
Yes, and it is built from shifts rather than one person staying indefinitely. Engagements range from a few hours a day for medication oversight and wound checks up to full 24-hour shifted nursing. No provider can staff round-the-clock coverage with a single nurse, and anyone implying otherwise is describing something that is not physically possible.
