Where good hospital coverage still leaves a gap at home
Ontario is unusually well served for acute care. Kaiser Permanente Ontario Medical Center is a 386,000 square foot hospital on South Vineyard Avenue, and San Antonio Regional Hospital, based in neighboring Upland, runs an outpatient location in the city. Strong hospital coverage does not translate into hours of skilled nursing at home, and that is where most Ontario families discover the gap.
Kaiser Permanente Ontario Medical Center
Kaiser's Ontario campus is a 386,000 square foot medical center at 2295 South Vineyard Avenue, open since November 2011, with emergency, cardiology, surgery and obstetrics, and a Leapfrog Group Top Hospital rating in 2024 and 2025. It handles the acute episode well. What no health plan covers is a nurse in the house for the twelve hours a day afterwards when a recovery is genuinely fragile.
An HMO covers the admission, not the hours at home
Families with Kaiser coverage are often surprised by this. Comprehensive plan membership does not include continuous private duty nursing at home, and neither does Medicare. Learning that in week one produces a very different plan than learning it in week three.
San Antonio Regional's Ontario site is urgent care, not the hospital
San Antonio Regional Hospital, founded in Upland in 1907, is a 363-bed nonprofit hospital with a full surgical, cardiac and emergency program at its 999 San Bernardino Road campus. Its Ontario Urgent Care Plus location on North Mountain Avenue handles urgent visits and outpatient behavioral health, not surgery or inpatient stays, so a family discharged from the Upland campus is coming from about six miles away, not from inside Ontario.
A young city with a fast-growing older population
Ontario's median age is 33.5 and only about 10 percent of residents are 65 or older, well under the county figure. But San Bernardino County's 65-and-older population is projected to grow 50 percent between 2025 and 2045, and a large share of Ontario's aging residents are long-time homeowners in the established neighborhoods around downtown, often sharing a home with the working-age family members building out Ontario Ranch nearby.

Ontario families discharge from two different kinds of surgical care: inpatient procedures at Kaiser Permanente Ontario Medical Center, outpatient elective cases at Ontario Advanced Surgery Center on West 6th Street, or from San Antonio Regional Hospital's 363-bed campus six miles away in Upland. Comfort Care Visit
Best for lower-acuity outpatient procedures, the visit type suited to same-day elective cases done at Ontario Advanced Surgery Center, a physician-owned ambulatory surgical center on West 6th Street. A Registered Nurse checks vital signs, reviews the discharge instructions with the family, sets up medications, and confirms wound care is being followed correctly. Also used as a standalone wellness check visit outside of a surgical context.
Full Recovery Day
Built for the day a patient comes home after a standard procedure at Kaiser Permanente Ontario Medical Center, the day risk is highest and the hospital's own care team is no longer watching. Covers vital sign monitoring, pain management coordination with the surgeon's office, mobility assistance, and medication administration per physician orders through a full first day home.
Elevated Recovery Day
The next step up for higher-acuity cases, spine surgery, joint revision, or the shoulder, back, and knee procedures common among the roughly 108,000 people working Inland Empire warehouse and logistics jobs, where a shorter hospital stay does not mean a lighter clinical need. Adds more intensive monitoring, drain management, and closer pain titration coordination with the surgical team across a longer day.
Overnight Recovery Care
For patients who want a private RN presence overnight at home rather than an unattended first night, whether the discharge came from Kaiser Permanente Ontario Medical Center or from San Antonio Regional Hospital's Upland campus, about six miles away. An RN stays through the night for symptom monitoring, medication timing, and mobility assistance if the patient needs to get up.
Round-the-Clock Care
Matched to hip fracture recovery and to older adults recovering alone in Ontario's established neighborhoods near downtown. Only about 10 percent of Ontario residents are 65 or older today, well under the county rate, but San Bernardino County's senior population is projected to grow 50 percent by 2045, and working-age family members in Ontario's logistics-driven economy often cannot be home during the day to provide it themselves. Neither hospital nor health plan puts a nurse in your parent's house for the twelve hours a day that a fragile recovery actually needs.
This catches Kaiser families in particular. Comprehensive HMO membership does not include continuous private duty nursing at home, and Medicare does not either: its home health benefit is limited to part-time or intermittent visits, generally under eight hours a day and 28 hours a week. Private duty nursing is the category that fills the remaining hours, and it is private pay.

Yes. Will Kaiser cover a private duty nurse at home after a discharge from Kaiser Permanente Ontario Medical Center?
No. Kaiser HMO membership covers the hospital admission at the 386,000 square foot Ontario Medical Center on South Vineyard Avenue and the plan's own follow-up services, but it does not fund continuous one-to-one nursing in your home. Neither does Medicare. Private duty nursing is private pay, though many long-term care insurance policies reimburse for it when the policy criteria are met.
What does post-surgical recovery care cost in Ontario?
It depends on the acuity and the hours needed, which is why we built six aftercare tiers instead of one flat rate. A single-visit Comfort Care check-in after an outpatient procedure covers 6 hours. A full first day home after standard surgery covers 8 hours, rising to 10 hours at a higher acuity level. Overnight RN coverage covers a 10-hour block, and round-the-clock care covers 24 hours. Extended, multi-day recovery stays are scheduled after intake once we understand the surgical plan.
