Why Pasadena

A practice built around San Gabriel Valley families

WholeHealth Concierge provides nurse-led care management, hospital-at-home support, and post-operative recovery oversight for Pasadena families. We coordinate directly with Huntington Hospital, USC Arcadia Hospital, and concierge physicians serving the San Gabriel Valley to bring hospital-level clinical attention into the home, without ever taking over the medical decisions that belong to your physician.

Coordination with Huntington Hospital

Huntington Hospital, part of Huntington Health and affiliated with Cedars-Sinai since 2020, operates a 534-bed facility at 100 W. California Boulevard and houses the only Level II trauma center serving the San Gabriel Valley, established in 1983. It also holds Joint Commission accreditation and advanced certification as a Comprehensive Stroke Center. We coordinate directly with its case management and discharge planning teams to arrange private-pay care before a patient leaves.

Coordination with USC Arcadia Hospital

USC Arcadia Hospital, the 348-bed community hospital at 300 W. Huntington Drive formerly known as Methodist Hospital of Southern California and now part of Keck Medicine of USC, has served San Gabriel Valley families for more than a century. It operates a certified stroke center and a STEMI heart attack receiving center alongside orthopedic, surgical, and maternity services. We work with its discharge planners the same way, so a nurse is already arranged before a patient goes home.

Care management for aging San Gabriel Valley families

Pasadena has one of the highest concentrations of long-tenured family homes in Southern California, and the surrounding communities skew older still: about 17.5 percent of Pasadena residents, 19.7 percent of Arcadia residents, and nearly a quarter of San Marino residents are 65 or older. Many of our clients are adult children coordinating care for a parent who has lived in the same home for decades. Our role is to make staying there medically safe.

Estate, elder law, and discretion

We work alongside Pasadena estate planning and elder law attorneys when a family is navigating capacity decisions, conservatorship considerations, or end-of-life care planning. The legal team handles the legal work; we provide the clinical nursing assessment and ongoing care oversight. Every visit, coordination call, and family communication is handled with the confidentiality San Gabriel Valley families expect.

Post-Surgical Aftercare, Matched to the Procedure

Six Levels of Recovery Care After Surgery in Pasadena

A 2007 Centers for Medicare and Medicaid Services report to Congress found that 17.6 percent of Medicare patients, roughly one in six, were rehospitalized within 30 days of discharge, the exact window post-surgical coverage is built to cover. Huntington Hospital's status as the San Gabriel Valley's only Level II trauma center and USC Arcadia Hospital's orthopedic, stroke, and cardiac services mean Pasadena-area families see everything from planned joint replacements to unplanned fracture repairs. We built six aftercare tiers so a family can match coverage to acuity instead of guessing. All tiers are private-pay and RN-delivered, coordinated with your surgeon's discharge plan rather than replacing it.

Tier 1

Comfort Care Visit

6 Hours

Best for lower-acuity outpatient procedures and post-op check-ins. A Registered Nurse checks vital signs, reviews the discharge instructions with the family, sets up medications, and confirms wound care is being followed correctly. Also used as a standalone wellness check visit for an aging parent outside of a surgical context.

Tier 2

Full Recovery Day

8 Hours

Built for the day a patient comes home from a standard joint replacement or general surgical procedure, the day risk is highest and the hospital's own case manager is no longer watching. Covers vital sign monitoring, pain management coordination with the surgeon's office, mobility assistance, and medication administration per physician orders through a full first day home.

Tier 3

Elevated Recovery Day

10 Hours

The next step up for higher-acuity cases, spine fusion, revision joint replacement, cardiac procedures, or multi-level surgery, where a shorter hospital stay did not mean a lighter clinical need. Adds more intensive monitoring, drain management, and closer pain titration coordination with the surgical team across a longer day.

Tier 4

Overnight Recovery Care

10-Hour Overnight Block

For joint replacement, spine, and cardiac patients who prefer an overnight stay at home rather than a facility, and for cosmetic and plastic surgery patients who want a private RN presence overnight. An RN stays through the night for symptom monitoring, medication timing, and mobility assistance if the patient needs to get up.

Tier 5

Round-the-Clock Care

24 Hours

Matched to the trauma and hip fracture cases Huntington Hospital's Level II trauma center sees, and to the share of San Gabriel Valley seniors living alone with no family member available to hand care off to. Continuous RN coverage across a full day, so care never steps down to unattended hours during the highest-risk window.

Tier 6

Extended Stay Recovery

Typically 48 to 72 Hours or Longer

For complex spine fusion, revision surgery, or plastic surgery patients with longer downtime windows, and for the multi-day recovery stretches common among Pasadena's aging, long-tenured homeowners. Scheduling and staffing are built around the individual recovery plan rather than a fixed block, so we quote it after intake.

All six tiers are private-pay. Medicare does not cover round-the-clock in-home care, homemaker support, or custodial recovery care, which is the gap this program is built to fill. Care reflects RN-delivered coverage coordinated with your surgeon's or physician's plan; we do not diagnose or independently treat, we execute the plan your medical team has already set. Pricing for each tier is confirmed by phone during a free consultation.

Private Duty Nursing

Private duty nursing in Pasadena

Pasadena and the San Gabriel Valley bring us a high share of complex, multi-condition cases, often after Huntington Hospital or USC Arcadia Hospital, formerly Methodist Hospital of Southern California, where several specialists are involved and no single one owns the plan.

Private duty nursing here is frequently paired with care management, because the nursing hours solve the night and the coordination solves the six appointments nobody is reconciling. It is also common in Altadena and other San Gabriel Valley communities still rebuilding after the January 2025 Eaton Fire, where older residents on fixed incomes have faced real pressure to relocate; keeping a recovering or aging family member safely at home is often part of the case for staying put.

Medicare does not cover round-the-clock home care, homemaker help, or custodial care, the exact gap that leaves families arranging it themselves at the moment they're least prepared to. We arrange it in advance instead, so coverage is already in place the day a patient walks in the door.

Pasadena Frequently Asked Questions

Common questions from Pasadena families

Do you serve all of Pasadena and the San Gabriel Valley?

Yes. We serve all of Pasadena including Old Pasadena, Madison Heights, Bungalow Heaven, Linda Vista, and San Rafael Hills. We also serve San Marino, Arcadia, South Pasadena, La Cañada Flintridge, Sierra Madre, Altadena, and surrounding communities.

What does private duty nursing cost after a discharge from Huntington Hospital or USC Arcadia Hospital?

It depends on what the recovery requires, which is why we built six aftercare tiers instead of one flat rate. A Comfort Care Visit covers six hours, a Full Recovery Day covers eight hours, and coverage scales up through Elevated Recovery Day, Overnight Recovery Care, and Round-the-Clock Care, which covers a full 24 hours. Extended Stay Recovery is built for multi-day recoveries and is scoped after consultation. All tiers are private-pay and RN-delivered, with pricing confirmed by phone during a free consultation.

Can you help after a Huntington Hospital discharge?

Yes. Hospital-at-Home transition support is one of our core services. Huntington Hospital operates the only Level II trauma center serving the San Gabriel Valley, and we coordinate directly with its case management and discharge planning teams to ensure the move home is medically supported and safe.

Do you work with concierge physicians in Pasadena?

Yes. We partner with concierge MDs, direct primary care practices, and specialist physicians serving Pasadena families. Your physician retains medical authority. We provide the in-home clinical execution and family coordination.

Do you work with Pasadena estate and elder law attorneys?

Yes. We provide the clinical nursing assessment and ongoing care oversight that supports estate planning, conservatorship, and capacity-related legal work. We do not provide legal advice.

How quickly can you start in Pasadena?

Intake assessments are typically completed within 24 to 48 hours. Urgent hospital-discharge situations are often handled the same day. Call (213) 298-3288 to speak with our care team.

Can an in-home nurse help an aging parent in San Marino, Arcadia, or Altadena who isn't recovering from surgery?

Yes. Nearly one in four San Marino residents and roughly one in five residents of Arcadia, Altadena, and Pasadena itself are 65 or older, and much of our work is ongoing rather than post-surgical. That means oversight of a complicated medication list, monitoring a chronic condition, catching small changes before they become a hospital visit, and coordinating between specialists so nothing falls through between appointments. Hours can be steady weekly coverage instead of a short recovery block, and every schedule is planned and supervised by a Registered Nurse.
