These Terms of Service ("Terms") apply to your use of the WholeHealth Concierge website and to the services we provide. By using this site or engaging our services, you agree to these Terms.
Our services
WholeHealth Concierge provides nurse-led private care management, including care assessments, hospital discharge planning and advocacy, post-operative recovery support, coordination of private duty nursing, and ongoing care coordination for families. Services are delivered and coordinated by California-licensed Registered Nurses and supporting staff.
Not emergency care, and not a substitute for your own providers
If this is an emergency, call 911. Our services are not emergency care, not urgent care, and not a replacement for your physician or care team. We coordinate with your providers; we do not replace them and we do not direct their medical decisions.
Not legal, financial, or tax advice
We advocate for clients and help families navigate systems, and we may work alongside attorneys, fiduciaries, and other professionals. We do not provide legal advice or legal representation, and we do not provide financial, insurance, or tax advice. Nothing on this website or said in the course of our work should be treated as any of those. Consult a qualified professional for those questions.
Information on this website
Articles, guides, and other content on this site are educational. They are general information, not medical advice about your situation, and reading them does not create a nurse-client or provider-patient relationship. Care regulations and benefit rules change, and content that was accurate when written may not stay accurate.
Engaging our services
Services begin under a separate written service agreement that sets out the scope, staffing, rates, and schedule for your engagement. These Terms govern the website and the general relationship. Where these Terms and your signed service agreement differ, the service agreement controls.
Scheduling and cancellation
Consultations and visits are booked through our scheduling tool or directly with our team. Cancellation notice periods and any associated charges are set out in your service agreement. We ask for as much notice as you can give so staffing can be adjusted.
Payment
Rates, minimums, and billing terms are set out in writing before services begin. We are a private-pay service. We do not bill Medicare, Medi-Cal, or health insurance on your behalf. Some clients are able to seek reimbursement from long-term care insurance or other benefits, and we can provide documentation, but we do not guarantee that any third party will pay or reimburse a claim.
No guarantee of outcomes
We bring clinical judgement, planning, and advocacy to a situation. We cannot and do not guarantee a particular medical outcome, a particular decision by a hospital, insurer, or facility, or a particular recovery timeline.
Your responsibilities
- Give us accurate and current information about the care situation, including medical history and medications, so that planning is safe.
- Tell us promptly when the situation changes.
- Ensure the person authorising care has the legal standing to do so.
- Provide a safe environment for staff working in a home.
Content you submit
If you submit a testimonial, review, photograph, or other content, you confirm it is yours to share and you give us permission to publish it in connection with our services. You can ask us to change or remove it at any time. Do not submit another person's health information without their permission.
Intellectual property
The content on this site, including text, guides, graphics, and the WholeHealth Concierge name and marks, belongs to WholeHealth Concierge or its licensors. You may read and share it for personal, non-commercial use with attribution. You may not republish it commercially without written permission.
Third-party links and tools
This site links to third-party resources and uses third-party tools for scheduling, messaging, and analytics. Those links are provided for convenience and reference. We do not control those sites or tools and are not responsible for their content, availability, or practices.
Limitation of liability
To the fullest extent permitted by law, WholeHealth Concierge is not liable for indirect, incidental, special, or consequential damages arising from your use of this website or the content on it. Nothing in these Terms limits any liability that cannot lawfully be limited, including liability for the professional conduct of licensed clinicians.
Governing law
These Terms are governed by the laws of the State of California, without regard to its conflict of law rules.
Changes to these Terms
We may update these Terms from time to time. The current version is always posted here with its effective date. Continuing to use the site after a change means you accept the updated Terms.
