A Trusted Clinical Partner

You should not have to manage a medical crisis alone.

When a loved one is discharged from the hospital, recovering from surgery, declining at home, or facing a complex medical condition, families are often left to coordinate everything on their own.

WholeHealth Concierge steps in as a trusted clinical partner to help organize the care plan, communicate with providers, coordinate the care team, monitor changing needs, and support the family through each decision. We bring structure, calm, and clinical oversight to moments that often feel overwhelming.

Concierge Nursing, Explained

What is concierge nursing?

Concierge nursing is private-pay, one-on-one care delivered by a registered nurse who works directly for your family rather than for a hospital or agency. A concierge nurse coordinates your doctors, manages medications and recovery, and stays available as a single, trusted point of contact, care built around your actual needs instead of insurance-approved visits.

The WholeHealth Method

A structured framework
for managing complex care at home.

Care management is not a series of disconnected services. It is a coordinated, nurse-led process that brings clarity, safety, and continuity to families during stressful medical moments.

01

Assess the Full Care Picture

We begin with a comprehensive review of medical history, current needs, home environment, family dynamics, and provider relationships. Families often come to us searching for a geriatric care manager; the difference here is that yours is a Registered Nurse.

02

Identify Risks and Urgent Needs

Discharge complications, medication concerns, safety risks, mobility limitations, and clinical red flags are identified early so they can be addressed before they escalate.

03

Create a Customized Care Plan

Each plan is built around the client's specific medical, personal, recovery, and family goals, not a generic care template.

04

Coordinate the Right Care Team

Licensed nurses, caregivers, companions, therapists, and specialists are matched, scheduled, and overseen based on real clinical need.

05

Communicate with Providers and Family

We act as the central point of clinical communication so families and physicians stay informed and aligned.

06

Monitor Changes and Adjust the Plan

Care needs change. We track progress, watch for clinical changes, and adjust the plan as conditions evolve.

07

Provide Ongoing Oversight and Support

Families receive ongoing clinical guidance, structured updates, and the reassurance of knowing a nurse is overseeing the plan from start to finish.

What We Manage

A complete care management approach.

WholeHealth Concierge is designed for families and individuals who need more than basic home care. We coordinate the clinical, personal, logistical, and family-facing details required to support safe, organized care at home.

Hospital-at-Home Program

Support for families who need more than a discharge plan.

A hospital discharge does not always mean care is simple. Many families leave the hospital with instructions, medications, equipment needs, follow-up appointments, and serious concerns about whether their loved one can safely recover at home.

WholeHealth Concierge helps bridge that gap with nurse-led planning, care coordination, private nursing support, caregiver oversight, provider communication, and ongoing monitoring of the client's changing needs.

  • Hospital-to-home transition planning
  • Discharge preparation and care needs review
  • Private nursing and care team coordination
  • Medication and care plan organization
  • Provider communication support
  • Home safety and recovery planning
  • Family updates and ongoing care oversight
  • Support for complex or high-acuity care needs

Frequently Asked Questions

Researched. Factual. From the field.

Direct answers to questions families and partners ask most often, grounded in current healthcare practice and research.

What is a nurse care manager?

A nurse care manager is a registered nurse who coordinates medical care across providers, settings, and family members. They assess clinical needs, build a written care plan, communicate with physicians, oversee caregivers, and adjust the plan as conditions change. The Aging Life Care Association recognizes nurse care management as a specialty within professional care management.

What is the difference between concierge nursing and home health?

Home health is short-term, physician-ordered, and typically covered by Medicare for specific post-discharge recovery periods. Concierge nursing is private-pay, ongoing, more comprehensive, and not dependent on insurance authorization. Concierge nursing allows for longer visits, proactive monitoring, and tighter family communication than home health agencies can provide under insurance reimbursement constraints.

Does Medicare cover concierge nursing or care management?

Original Medicare does not cover private concierge nursing or care management. Some Medicare Advantage plans cover limited supplemental benefits but rarely the full scope of concierge services. Concierge care is typically paid privately, either out of pocket or through long-term care insurance policies.

How quickly can WholeHealth Concierge begin services?

Initial consultations are typically scheduled within 24 to 48 hours. Onsite assessments can usually be completed the same week, with a written care plan delivered shortly after. Urgent cases involving hospital discharge or post-surgical recovery are prioritized.

What geographic areas do you serve?

WholeHealth Concierge serves families across Orange County, Los Angeles County, San Bernardino County, and parts of Riverside County. Specific zip code coverage and any travel considerations are discussed during the initial consultation.
