Why Irvine

A practice built around Irvine families

WholeHealth Concierge provides nurse-led care management, post-operative recovery oversight, and hospital-to-home transition support for Irvine families. We work with concierge physicians, surgeons, and hospital discharge teams to bring hospital-level clinical attention into the home, without ever taking over the medical decisions that belong to your physician.

Coordination with Hoag Hospital Irvine

Hoag Hospital Irvine's Sun Family Campus is in the middle of a $1 billion expansion, funded in part by a $100 million gift from the Sun family, adding six new buildings and 155 inpatient beds. The campus's new Surgical Pavilion opened in May 2026 with eight operating rooms, including a hybrid OR for combined imaging and surgery. Hoag is rated the highest-ranked hospital in Orange County by U.S. News & World Report for 2025 to 2026. We coordinate directly with case management and discharge planning there to arrange private-pay care before a patient leaves.

Recovery after Hoag Orthopedic Institute procedures

Hoag Orthopedic Institute, adjacent to the hospital on Sand Canyon Avenue, has the highest hip, knee, and shoulder replacement volume on the West Coast and passed 300,000 spine and orthopedic procedures in March 2026. It is the only Orange County hospital named a U.S. News Best Hospital for orthopedics, rated High Performing in Hip Replacement, Knee Replacement, Hip Fracture, and Spinal Fusion. In-home post-surgical recovery is coordinated with the surgical team on wound care, mobility, pain management, and rehab follow-through.

UCI Health's Irvine facilities

UCI Health's Irvine footprint includes the Joe C. Wen & Family Center for Advanced Care, the Gavin Herbert Eye Institute, and the new UCI Health Irvine Rehabilitation Hospital, a 52-bed facility built with Lifepoint Rehabilitation for stroke, spinal cord injury, and post-surgical mobility recovery. We coordinate hospital-to-home and rehab-to-home transitions with these teams for Irvine families discharged from any of them.

Care management for Irvine's high-net-worth families

Irvine is ranked the third-richest city in America, with a median household income of $136,719 and an average household income of $175,308. We provide the nurse-led equivalent of a personal medical concierge: one point of contact with the clinical authority to actually deliver the care, not just schedule it, for a recovering family member or an aging parent.

Post-Surgical Aftercare, Matched to the Procedure

Six Levels of Recovery Care After Surgery in Irvine

Same-day discharge is now the majority preference after joint replacement: 63 percent of total joint arthroplasty patients say they would rather go home the day of surgery than stay overnight. Outpatient adoption varies sharply by procedure, from roughly 71 percent for ankle surgery down to about 1 percent for hip replacement, so what a patient needs at home on day one looks very different depending on what was actually done. Enhanced Recovery After Surgery protocols, now standard at high-volume orthopedic programs like Hoag Orthopedic Institute, have shortened hospital stays even further, which compresses the window families have to arrange care before someone walks in the door needing it. We built six aftercare tiers so a family can match coverage to acuity instead of guessing. All tiers are private-pay and RN-delivered, coordinated with your surgeon's discharge plan rather than replacing it.

Tier 1

Comfort Care Visit

6 hours

Best for lower-acuity outpatient procedures and post-op check-ins, the visit type that fits the roughly 70 percent of ankle procedures and half of shoulder procedures now done same-day. A Registered Nurse checks vital signs, reviews the discharge instructions with the family, sets up medications, and confirms wound care is being followed correctly. Also used as a standalone wellness check visit outside of a surgical context.

Tier 2

Full Recovery Day

8 hours

Built for the day a patient comes home from a standard joint replacement at Hoag Orthopedic Institute, the day risk is highest and the surgical team's case manager is no longer watching. Covers vital sign monitoring, pain management coordination with the surgeon's office, mobility assistance, and medication administration per physician orders through a full first day home.

Tier 3

Elevated Recovery Day

10 hours

The next step up for higher-acuity same-day cases, spine fusion, revision joint replacement, or multi-level procedures, where a shorter hospital stay did not mean a lighter clinical need. Adds more intensive monitoring, drain management, and closer pain titration coordination with the surgical team across a longer day.

Tier 4

Overnight Recovery Care

10 overnight hours

For the roughly 25 percent of joint replacement patients who prefer or require an overnight stay, and for cosmetic and plastic surgery patients who want a private RN presence overnight at home rather than a surgical recovery facility. An RN stays through the night for symptom monitoring, medication timing, and mobility assistance if the patient needs to get up.

Tier 5

Round-the-Clock Care

24 hours

Matched to hip fracture recovery, a category where Hoag Orthopedic Institute is rated High Performing by U.S. News, and to the roughly 35 percent of Irvine adults 65 and older who live alone with no family member available to hand care off to. Continuous RN coverage across a full day, so care never steps down to unattended hours during the highest-risk window.

Tier 6

Extended Stay Recovery

48 to 72 hours or longer

For complex spine fusion, revision surgery, or plastic surgery patients with longer downtime windows, and for the multi-day or multi-week recovery stretches that become more common as Orange County's senior population grows toward a projected 17.3 percent of residents by 2045. Scheduling and staffing are built around the individual recovery plan rather than a fixed block, so we quote it after intake.

All six tiers are private-pay. Medicare does not cover round-the-clock in-home care, homemaker support, or custodial recovery care, which is the gap this program is built to fill. Care is RN-delivered and coordinated with your surgeon's or physician's plan; we do not diagnose or independently treat, we execute the plan your medical team has already set. Pricing for each tier is confirmed by phone during a free consultation.

Private Duty Nursing

Private duty nursing in Irvine

Irvine families tend to come to private duty nursing through a planned event rather than a crisis: a scheduled surgery, a parent moving in, or a discharge from Hoag Hospital Irvine or UCI Health's Irvine facilities with orders nobody at home feels qualified to carry out.

That planning window is an advantage. When we have notice, we can match a nurse to the specific clinical need instead of whoever is available, and stage coverage heaviest in the first week home, when readmission risk is highest. Nearly one in five older adults nationally is treated again in a hospital within 30 days of leaving one, and nurse-led transition support is one of the few things shown to bring that number down.

Medicare does not cover round-the-clock home care, homemaker help, or custodial care, the exact gap that leaves families arranging it themselves at the moment they're least prepared to. We arrange it in advance instead, so the coverage is already in place the day a patient walks in the door.

Irvine Frequently Asked Questions

Common questions from Irvine families

Do you serve all of Irvine?

Yes. We serve every Irvine neighborhood, including Turtle Rock, Quail Hill, Woodbridge, Northwood, Shady Canyon, Orchard Hills, University Park, and Westpark, along with nearby Newport Beach and Lake Forest.

What does private nursing cost after surgery at Hoag Orthopedic Institute or Hoag Hospital Irvine?

It depends on what the recovery requires, which is why we built six aftercare tiers instead of one flat rate. A Comfort Care Visit covers six hours, a Full Recovery Day covering the day of a standard joint replacement discharge covers eight hours, and coverage scales up through Elevated Recovery Day, Overnight Recovery Care, and Round-the-Clock Care, which covers a full 24 hours. Extended Stay Recovery covers multi-day recoveries of 48 to 72 hours or longer. All tiers are private-pay and RN-delivered, and pricing for each tier is confirmed by phone during a free consultation.

Do you partner with concierge physicians in Irvine?

Yes. We work alongside concierge MDs, direct primary care practices, and specialist physicians serving Irvine families. In-home clinical execution is coordinated by Meagan Williams, RN, BSN, with CCRN certification in adult critical care, and the nurses she works with, while your physician retains medical authority over the care plan.

How quickly can you start care in Irvine, including for a same-day hospital discharge?

Intake assessments are typically completed within 24 to 48 hours. Because 63 percent of joint replacement patients now go home the same day as surgery, we regularly arrange coverage to start the moment a patient leaves the hospital or surgery center. Call (213) 298-3288 to speak with our care team about timing.

Can you support families across cultures and languages?

Yes. We coordinate care for Irvine's multilingual community and make sure aging parents, family members, and caregivers can all understand the care plan and follow it correctly.

How do I find an in-home nurse in Irvine after surgery?

Call before your family member leaves the hospital if there's still time. We complete an intake assessment, usually within 24 to 48 hours and often the same day for an urgent discharge, then match a nurse to what the recovery actually requires: wound care, drain monitoring, medication oversight, and reporting back to the surgeon. Coverage is arranged so a licensed nurse is already in place the day your family member gets home.

Can an in-home nurse help an aging parent who lives alone and isn't recovering from surgery?

Yes. Roughly 35 percent of Irvine adults 65 and older live alone, and a large share of our work is ongoing rather than post-surgical: oversight of a complicated medication list, monitoring a chronic condition, catching small changes before they become a hospital visit, and coordinating between specialists so nothing falls through between appointments. Hours can be steady weekly coverage instead of a short recovery block, and every schedule is planned and supervised by a Registered Nurse.
