A coordinated care team, guided by clinical oversight.
WholeHealth Concierge does not simply place caregivers into the home. We help identify, coordinate, and oversee the appropriate level of support based on each client's condition, goals, family dynamics, and care environment.
Depending on the client's needs, the care team may include licensed nurses, vocational nurses, caregivers, companions, therapists, respiratory support, dialysis support, physicians, specialists, attorneys, fiduciaries, and other trusted professionals.
Find the right program for your family.
Each care situation is unique. We help families understand which level of support is appropriate, what the care plan should include, and how to move forward with confidence.
Researched. Factual. From the field.
Direct answers to questions families and partners ask most often, grounded in current healthcare practice and research.
What is typically included in a concierge nursing engagement?
Engagements typically include initial assessment, written care plan, provider coordination, medication review, family communication, caregiver oversight, appointment attendance, hospital discharge support, and ongoing clinical monitoring. The exact scope is customized for each client.
Do you provide hands-on nursing care or coordination only?
Both are available. Hands-on services like IV therapy, post-surgical wound oversight, and skilled nursing visits are delivered when needed. Care coordination, advocacy, and care management run continuously across the engagement.
How is concierge nursing billed?
Services are billed privately, typically through hourly rates, retainer packages, or project-based engagements. Specific pricing is discussed during the consultation based on scope of need.
Can we use concierge nursing alongside an existing home health agency?
Yes. Many families layer concierge nursing on top of Medicare-covered home health to add advocacy, coordination, and continuity that insurance-reimbursed agencies cannot provide under their reimbursement constraints.
Do you accept long-term care insurance?
Some long-term care policies will reimburse for nurse-led care management. We can help families review the policy and submit appropriate documentation when a covered benefit applies.
