A Home Nurse in Chino Hills, Where Our Practice Is Based
WholeHealth Concierge is headquartered in Chino Hills, a hillside Inland Empire city of roughly 77,000 residents where about 13 percent of the population is 65 or older. We provide RN-led care management, hospital-at-home support, and private duty nursing coordination, staged from a single visit up through 24-hour coverage, for Chino Hills and Inland Empire families. We respond quickly across the Chino and Pomona valleys, with bilingual coordination for multigenerational households. Founded and led by Meagan Williams, BSN, CCRN certification in adult critical care, with 15+ years in critical care nursing.
Now serving Diamond Bar, Pomona, Chino, Walnut, Rowland Heights, and Yorba Linda.
A practice built around Inland Empire families
WholeHealth Concierge is headquartered in Chino Hills and provides nurse-led care management, hospital-at-home support, and private duty nursing coordination for families across the Inland Empire. Meagan Williams, BSN, CCRN certification in adult critical care, founded the practice here to bring hospital-level clinical oversight into the home for patients who deserve more than what a traditional home health agency provides.
Headquartered here, not just covering the area
Our office is in Chino Hills, which keeps response times short across the Chino and Pomona valleys and the wider Inland Empire. Same-day intake assessments are routinely available for families navigating a hospital discharge or a sudden change in a parent's condition.
Close to the hospitals your family already knows
Chino Hills does not have a full-service hospital of its own inside the city limits. Most of our clients here come to us through a discharge from Chino Valley Medical Center in Chino, Pomona Valley Hospital Medical Center in Pomona, San Antonio Regional Hospital in Upland, or Kaiser Permanente Ontario Medical Center. We coordinate directly with the discharge planning teams at each of these facilities.
Built for Chino Hills' hillside homes
Many Chino Hills homes sit on elevated lots with multiple flights of interior stairs, common in neighborhoods like Vellano, Eagle Glen, and Los Serranos. That layout is part of what raises the stakes in the first days after a joint replacement or a fall, and it is one of the first things an RN evaluates during intake.
Serving multigenerational, multilingual households
Chino Hills is one of the more ethnically diverse cities in the Inland Empire, with Asian residents (about 41 percent) and Hispanic residents (about 27 percent) together making up the majority of the population, many in multigenerational households. When a family member speaks limited English, we coordinate care plans, medication schedules, and physician communications so the whole household is aligned on what is happening and what to do next.
Six core programs, one nurse-led team
Every Chino Hills engagement starts with an intake assessment and a written care plan led by a Registered Nurse. From there we coordinate the services that fit the situation.

San Antonio Regional Hospital in Upland holds The Joint Commission's Gold Seal of Approval for Advanced Total Hip and Knee Replacement Certification, and Chino Valley Medical Center runs its own orthopedic and surgical program about five miles from Chino Hills. Nationally, in a survey of patients who had experienced both, 63 percent said they preferred same-day discharge after a total joint replacement over an overnight hospital stay. That preference means more patients are walking back into a Chino Hills home, often up a flight of stairs, on the same day as their surgery, with no clinical staff left to catch a problem overnight. At the same time, San Bernardino County's 65-and-older population is projected to grow from about 13.6 percent of residents in 2025 to 20 percent by 2045, the fastest-growing age group in the county. We built six aftercare tiers so a family can match coverage to what the surgery actually requires instead of guessing. All tiers are private-pay and RN-delivered, coordinated with your surgeon's discharge plan rather than replacing it.
Comfort Care Visit
Best for lower-acuity outpatient procedures and post-op check-ins done at San Antonio Regional Hospital's Joint Replacement Center or a same-day surgical center. A Registered Nurse checks vital signs, reviews the discharge instructions with the family, sets up medications, and confirms wound care is being followed correctly. Also used as a standalone wellness check visit outside of a surgical context.
Full Recovery Day
Built for the day a patient comes home from a standard knee or hip replacement, the day risk is highest and the surgical team's case manager is no longer watching. Covers vital sign monitoring, pain management coordination with the surgeon's office, mobility assistance on Chino Hills' hillside stairs and driveways, and medication administration per physician orders through a full first day home.
Elevated Recovery Day
The next step up for higher-acuity same-day cases, spine fusion, revision joint replacement, or multi-level procedures, where a shorter hospital stay did not mean a lighter clinical need. Adds more intensive monitoring, drain management, and closer pain coordination with the surgical team across a longer day.
Overnight Recovery Care
For patients who prefer or require an overnight stay after joint replacement, and for cosmetic and plastic surgery patients who want a private RN presence overnight at home rather than a surgical recovery facility. An RN stays through the night for symptom monitoring, medication timing, and mobility assistance if the patient needs to get up, which matters in hillside homes with stairs between the bedroom and the bathroom.
Round-the-Clock Care
Matched to hip fracture recovery and to older adults in Chino Hills' hillside neighborhoods who live alone with no family member nearby to hand care off to. Continuous RN coverage across a full day, so care never steps down to unattended hours during the highest-risk window.
Extended Stay Recovery
For complex spine fusion, revision surgery, or plastic surgery patients with longer downtime windows, and for the multi-day recovery stretches that will become more common as San Bernardino County's senior population climbs toward a projected one in five residents by 2045. Scheduling and staffing are built around the individual recovery plan rather than a fixed block, so we quote it after intake.
Private duty nursing in Chino Hills
Chino Hills is home. WholeHealth Concierge is headquartered here, which means private duty nursing in the Chino and Pomona valleys is a short drive rather than a scheduled trip, and shift coverage gets filled faster than it does at the edges of our service area.
Most local private duty cases begin at a discharge desk. Families come to us from Chino Valley Medical Center, Pomona Valley Hospital Medical Center, and San Antonio Regional Hospital in Upland, usually with the same problem: the discharge plan assumes someone competent will be at the house, and Medicare home health tops out around 28 hours a week. Private duty nursing is what covers the gap between those two facts.
We cover the neighboring Inland Empire cities from the same base: Ontario, Upland, Rancho Cucamonga, Eastvale and Corona.

Do you serve all of Chino Hills?
Yes. We serve all of Chino Hills, including North and South Chino Hills neighborhoods such as Vellano, Eagle Glen, Los Serranos, Carbon Canyon, Soquel Canyon, and Sleepy Hollow. We also serve the surrounding Inland Empire, including Diamond Bar, Chino, Pomona, Walnut, Rowland Heights, and Yorba Linda.
What hospitals do you coordinate with for patients in Chino Hills?
Chino Hills does not have a full-service hospital within its own city limits, only Pomona Valley Health Center clinic locations and an urgent care. Most of our Chino Hills clients come to us through a discharge from Chino Valley Medical Center in Chino, about five miles away, Pomona Valley Hospital Medical Center in Pomona, San Antonio Regional Hospital in Upland, which holds Joint Commission certification for total hip and knee replacement, or Kaiser Permanente Ontario Medical Center. We coordinate directly with the discharge planning team at each of these facilities.
How much does private duty nursing or post-surgical aftercare cost in Chino Hills?
Our post-surgical aftercare is structured into six tiers, so a family pays for the level of coverage the procedure actually requires, from a short Comfort Care Visit up through full 24-hour Round-the-Clock Care. Longer, complex recoveries such as an Extended Stay Recovery are priced after a consultation. All care is private-pay, and exact rates are confirmed by phone during a free consultation. Medicare does not cover round-the-clock home care, custodial care, or homemaker support, which is the gap this program exists to fill.
How quickly can you start care in Chino Hills?
For most situations we can complete an intake assessment within 24 to 48 hours and begin coordinating care immediately. Because our practice is headquartered in Chino Hills, urgent hospital discharge situations across the Chino and Pomona valleys are often handled the same day. Call (213) 298-3288 to speak with our care team.
Chino Hills has a lot of multigenerational households. Can you coordinate care in more than one language?
Yes. Chino Hills is one of the more ethnically diverse cities in the Inland Empire, with a large share of Asian and Hispanic households, many of them multigenerational. When a family member speaks limited English, we coordinate care plans, medication schedules, and physician communications so the whole household understands what is happening and what to do next.
Our home has stairs and a hillside driveway. Does that change the care plan after surgery?
It should. Many Chino Hills homes, particularly in hillside neighborhoods like Vellano, Eagle Glen, and Los Serranos, sit on elevated lots with multiple flights of interior stairs and steep driveways. That layout raises fall risk in the first days after a joint replacement or any procedure that limits mobility. An RN evaluates the home layout during intake and builds the mobility and supervision plan around it, not around a generic recovery timeline.
Do you accept long-term care insurance in the Inland Empire?
We are a private-pay practice, but most long-term care insurance policies will reimburse families directly for private duty nursing and aftercare. We provide itemized invoices and documentation to support your reimbursement claim.
